Testosterone: what therapeutic trials show, and where physique dosing departs from them

Testosterone has the strongest human evidence base in this field, but almost all of it comes from replacement dosing in men with low testosterone. Physique use typically involves several times those doses, where the trial evidence no longer applies.

What the literature shows

Classic dose-response work showed graded increases in lean mass and strength across escalating testosterone doses in healthy men, alongside graded adverse changes in lipids and haematocrit.

Large replacement trials in men with low testosterone report symptomatic benefit with a defined safety profile at therapeutic doses.

Cohort and imaging studies of long-term illicit users report impaired cardiac function, increased plaque burden and persistent hormonal suppression.

Fertility literature documents azoospermia during exogenous testosterone use, with variable and sometimes prolonged recovery after cessation.

What remains unknown

  • Long-term cardiovascular outcomes at supraphysiological doses have never been trialled and cannot ethically be.
  • Predictors of slow or incomplete hormonal recovery are not well defined.
  • Optimal monitoring intervals outside clinical care are unestablished.

Safety and monitoring implications

  • Haematocrit, blood pressure, lipids and prostate-relevant markers are the core monitoring set; raised haematocrit is common and clinically meaningful.
  • Fertility should be treated as suppressed during use, regardless of intent.
  • Extrapolating replacement-trial safety data to physique doses is the single most common reasoning error in this area.
